8i bug: Found dead multi-thread server

"Found dead multi-thread server" -- this error occurs in our database and causes our instance to crash. Nothing else. No other error.

- Database info: Oracle8i Enterprise Edition Release 8.1.6.0.0 (running on Windows 2000 Advanced Server)

I have no idea what causes the problem.
This sounds like bug #2622690. This bug is fixed in Oracle10g.
